রক্তে প্লাটিলেট বা অণুচক্রিকার স্বাভাবিক মাত্রা দেড় লাখ থেকে সাড়ে চার লাখ। ডেঙ্গুতে প্লাটিলেট কমা স্বাভাবিক। প্রায় ৯০ শতাংশ রোগীর প্লাটিলেট কমে। তবে তা এক লাখের নিচে নামে না। এ ছাড়া নানা ধরনের ভাইরাস জ্বরেও প্লাটিলেট কমতে পারে। তবে ডেঙ্গুতে আক্রান্ত ১০ থেকে ২০ শতাংশ রোগীর ক্ষেত্রে বিষয়টি জটিল আকার ধারণ করে। ৫ শতাংশ ডেঙ্গু রোগীর মারাত্মক রক্তক্ষরণ হয়। এ ক্ষেত্রে মনে রাখতে হবে, প্লাটিলেট ২০ হাজারের নিচে নামলেই কেবল রক্তক্ষরণের ভয় থাকে।
আর তা যদি ৫ হাজারের কম হয়, তাহলে মস্তিষ্ক, কিডনি, হৃদ্যন্ত্রে রক্তক্ষরণের ঝুঁকি থাকে। রক্তক্ষরণের কারণে যদি রক্তচাপ কমে যায়, হৃৎস্পন্দন বেড়ে যায়, হিমোগ্লোবিন ১০–এর কম হয়, তবেই রক্ত দেওয়ার প্রশ্ন আসে। তবে রক্ত দিলে প্রতি ইউনিটে রোগীর প্লাটিলেটের সংখ্যা বাড়ে মাত্র ২০ হাজার। কাজেই এতে খুব একটা যে লাভ হয়, তা–ও নয়। হিসাবমতে, ডেঙ্গুর চিকিৎসায় প্লাটিলেটের দরকারই পড়ে না। রোগীর রক্তে প্লাটিলেটের সংখ্যা ডেঙ্গুর তীব্রতার মাত্রা বুঝতে সাহায্য করে।
এবারের ডেঙ্গু আলাদা কেন
বিশ্ব স্বাস্থ্য সংস্থার তথ্যমতে, ডেঙ্গু হেমোরেজিক ফিভারে রক্তে প্লাটিলেটের সংখ্যা এক লাখের নিচে নেমে যায় বা হেমাটোক্রিট (রক্তের মোট পরিমাণের সঙ্গে লোহিত কণিকার মোট পরিমাণের অনুপাত) ২০ শতাংশের বেশি বেড়ে যায়। এর সঙ্গে রক্তনালিতে রক্তপাতের কারণে প্রোটিন কমে যাওয়া, বুকে-পেটে পানি জমাসহ অন্যান্য জটিলতাও দেখা দেয়। সাধারণত দ্বিতীয় বা তৃতীয়বার আক্রান্ত হলে ডেঙ্গু হেমোরেজিক ফিভার হয়। বাংলাদেশে ডেঙ্গুর প্রাদুর্ভাব শুরু হয়েছে ২০০০ সাল থেকে। কাজেই এবার অনেকেই দ্বিতীয় বা তৃতীয়বার আক্রান্ত হচ্ছেন।
ডেঙ্গু হেমোরেজিক ফিভার আবার দুই ধরনের। হেমোরেজিক ফিভার গ্রেড-১ সাধারণ ডেঙ্গু জ্বরের মতোই। রক্তক্ষরণ হয় না, কেবল টোরনিকেট পরীক্ষায় ত্বকের নিচে রক্তবিন্দু দেখা যায়। গ্রেড-২ হলে চোখ, নাক, ত্বকের নিচে রক্তক্ষরণ হয়। সতর্কসংকেত হলো বমি, পেট ব্যথা, খাদ্যনালি, মূত্রনালি, মেয়েদের যোনিপথে রক্তক্ষরণ এবং এগুলো ক্রমশ বাড়তে থাকা। এমনটা হলে অবশ্যই হাসপাতালে ভর্তি হতে হবে।
ডেঙ্গু কখন মারাত্মক
বিশ্ব স্বাস্থ্য সংস্থা ডেঙ্গুকে নতুন করে সংজ্ঞায়িত করেছে। সে অনুযায়ী ডেঙ্গু তিন প্রকার। ১. সতর্কসংকেতহীন ডেঙ্গু ২. সতর্কসংকেতসহ ডেঙ্গু এবং ৩. মারাত্মক বা সিভিয়ার ডেঙ্গু
জ্বর কমে যাওয়ার পর অর্থাৎ রোগের ষষ্ঠ, সপ্তম বা অষ্টম দিনে সিভিয়ার ডেঙ্গু দেখা দেয়। এ সময় রক্তচাপ কমে যায়, রক্তবমি হয়, কালো মল, প্রস্রাবের সঙ্গে রক্ত যেতে পারে, ত্বকের নিচে রক্ত জমে, প্রস্রাব কমে যায়, যকৃৎ বড় হয়ে যায়। এ ছাড়া বুকে–পেটে পানি জমে, ফুসফুসে পানি জমে শ্বাসকষ্ট হয়। রক্তচাপ অনেক কমে যাওয়ায় অস্থিরতা, অসংলগ্ন কথাবার্তা, একাধিক অঙ্গ অকার্যকর হয়ে পড়তে পারে।
প্রথম থেকে ঠিকমতো চিকিৎসা করালে এবং সঠিক পরিমাপে তরল পদার্থ দিতে পারলে ডেঙ্গু মারাত্মক আকার ধারণ করে না। রোগীকে নিবিড় পর্যবেক্ষণে রেখে চিকিৎসা দিতে পারলে ডেঙ্গু সম্পূর্ণ ভালো হয়ে যায়।
